Patients are asked to fill in a registration sheet and a consent declaration, which you can print out beforehand. Your insurance card is also necessary.
If current x-rays or models exist, it would be helpful if you can bring them along to the first appointment. If you wish we can request it, if necessary, from your dentist or orthodontist.
